  1. Your Age?

  2. What areas of the scalp do you want improved (front, middle, crown)?

  3. Any medical conditions?

  4. Any medications?

  5. Are you on Rogaine or Propecia, and if so, for how long?

  6. Any prior hair transplant procedures? If so, when, how many grafts, and in what areas?
